Job Coach -
As a Job Coach/Program Trainer for Community Employment Services, it is critical that you provide support services to individuals with disabilities that are customized to their needs and preferences while promoting successful competitive employment outcomes for consumers. You will be expected to provide supports and services to consumers working at various employment sites in the community. This support must be based on the individual consumer’s needs and centered around work-related goals that focus on helping the consumer obtain the maximum level of independence within their job. You will be expected to be an offsite representative of My Day Counts, which comes with the responsibilities of representing the organization in a professional and positive manner with all stakeholders as well as acting in a way that is a reflection of the agency’s standards of excellence. It is critical that you consistently produce professional and meaningful progress notes that meet the standards of funding agents, accreditation entities, and My Day Counts. It is essential that your work schedule is flexible to meet the support needs of assigned consumers and be available to act as an advocate on and off the job site.


• Have at least one (1) year experience working with adults with disabilities and/or disadvantaged populations.
• Have a high school diploma or GED equivalent.
• Be available for work based on consumer needs, which can include evenings and weekends.
• Be able to work effectively within a team dynamic as well as independently and with autonomy.
• Have the ability to prioritize and organize, be detail-oriented, demonstrate necessary writing skills to produce professional documentation, and have computer skills necessary to perform job duties.
• Be at least eighteen (18) years of age.
• United States Citizenship or evidence of a valid work permit.
• Must successfully complete required D.O.J. Live Scan and criminal history background review by the State of California and be free of any felony convictions.
• Must successfully complete a health screening, physical examination, and TB test.
• Possess a valid California Driver’s License, a DMV report satisfactory to the agency’s insurance carrier, and proof of automobile liability insurance coverage as required by law.
• Be committed to and evidence support for the agency’s Mission of “Making life better for people with disabilities”.


• Provide customized support and training that promotes the retention of the consumer’s competitive employment placement.
• Provide customized support and training that promotes the consumer having successful paid internship outcomes.
• Consistently model positive and appropriate professional working relationships and work maturity skills, and hold consumers to the same expectations.
• Address employer concerns proactively and professionally so effective resolution is reached that successfully maintains the consumer’s employment.
• Maintain an atmosphere that is welcoming, understanding, communicative, and supportive of consumers.
• Monitor consumer’s work performance and provide constructive feedback and prompts when needed.
• Employ the use of person-centered planning so that consumer choice and preferences are evident in service provision and in supporting the consumer to reach his/her program plan goals.
• Ensure quality and safe delivery of all program supports and services at all times, including the safe use and maintenance of worksite equipment, materials and products.
• Advocate for services and supports that will enhance the quality of life and employment opportunities for consumers.
• Responsible for recognizing and reporting emergencies and changes in consumer status, including consumer’s physical condition, behavior, health, functioning, or appearance to the Program Coordinator or Director.
• Represent My Day Counts in a professional and positive manner with all stakeholders.
• Adhere to the department’s requirements for the percentage of time spent in billable activities.
• Produce all required documentation in a professional and timely manner; ensuring the documentation meets the standards established by funding agents, accreditation entities, program design. and Client.
• Adhere toClient Code of Ethical Conduct.
• Be familiar with all HIPAA and HITECH regulations and employ safeguards as directed by HIPAA and HITECH laws and State of California and Client privacy regulations.
• Provide support and perform assigned team duties during an emergency or catastrophic event as defined in Client Emergency Plan.
• Responsible for the implementation and execution of Client safety procedures and guidelines.
• Advocate for services and support that will enhance the quality of life for clients.
• Other duties as assigned to contribute to the overall effectiveness of the organization and program.
